Patient Registry Assistance into the Medical Cannabis Pilot Program

On August 1, 2013, the Compassionate Use of Medical Cannabis Pilot Program Act ("Act"), Public Act 98-0122, became effective.  The Medical Cannabis Patient Registry Program allows persons who are diagnosed with a qualifying debilitating medical condition to register with the Illinois Department of Public Health (IDPH) in order to obtain access to cannabis (marijuana) for medical use.  Currently 28 states and Washington, DC have legalized the medical use of cannabis. The Illinois Department of Public Health has authorized certified local health departments (LHD) to provide assistance to qualifying patients with completing their applications for the Medical Cannabis Registry Program.

An application assistant will guide you through the steps and what is needed to complete all parts of the application:

  • Physician Written Certification Form- your doctor must complete this form within 90 days of your application appointment
  • Application Fee to the State of Illinois- select a one, two, or three-year fee (pay online during appointment)
  • Photograph- 2x2 inch passport-sized photo (can be taken at appointment)
  • Proof of Age & Identity- valid, unexpired driver's license or government issued photo ID
  • Fingerprint Clearance—the application assistants can help you set up an appointment with a fingerprinting service and complete the Consent Form that will need the transaction number (TCN) after fingerprinting is completed

*Patient can designate a caregiver to assist with the use and purchase of medical cannabis. Caregiver will need to complete an application along with the patient.
