The DeKalb County Health Department, a department of county government, was founded in 1968. The Health Department offers a full array of public health programs and is certified by the Illinois Department of Public Health.

Flu Vaccine is Now Available
Flu vaccination is now available for adults and children 6 months and older on a walk-in, first-come, first-served basis. No appointment is needed.
Visit us during regular clinic hours Monday-Friday between 8:00am-4:00pm to receive your flu vaccine.
Please bring all insurance cards and a valid photo ID. No insurance? No problem. Flu vaccines are $38.

Northwestern Medicine is conducting a Community Health Survey and the DeKalb County Health Department (DCHD) wants to hear from you, too!
You do not need to be an NM patient to participate. Your answers will help DCHD better understand what our community health needs are and help guide our IPLAN planning process.
If you’re 18+ and live in a community served by Northwestern Medicine, take the survey by October 23 to make sure your voice is heard.
Vax Verify – Access Your Vaccination History
Illinois residents can now securely download their full immunization history directly to their mobile devices.
To access Vax Verify, go to the IDPH Portal. Once you complete the brief identity verification process, you’ll be able to view and download your official immunization record.
