Only certified copies of birth records of persons who were born in DeKalb County, Illinois may be obtained at the DeKalb County Health Department.
IN PERSON
Birth certificates may be obtained at DeKalb County Health Department Monday thru Friday from 8:00 am to 4:00 pm on a walk-in basis. Please bring a valid government issued photo ID. Alternatively, see the link below for other acceptable forms of identification. We accept cash, check, money order and major credit cards.

- Print and sign the payment confirmation.
- Email a photo of your signed confirmation and a copy of your valid government photo ID to: vitalrecords@dekalbcounty.org

Complete the Birth Certificate Request form, attach a copy of your valid government issued photo ID and enclose payment in the form of check or money order payable to the DeKalb County Health Department.

Who Can Obtain a Birth Record?
Illinois Statutes provide that copies of birth records may be obtained only by the person himself/herself (if of legal age), a parent, or other legal representative with a valid acceptable identification (government-issued), (410 ILCS 535/25).

Credit Card Fees:
Effective 01/04/2024, a credit card fee of 2.25% will be applied to all debit and credit card transactions. A minimum of $1.00 will be applied.
