Only certified copies of death records of persons who died in DeKalb County, Illinois may be obtained at the DeKalb County Health Department.
IN PERSON
Death certificates may be obtained at DeKalb County Health Department Monday thru Friday from 8:00 am to 4:00 pm on a walk-in basis. Please bring a valid government issued photo ID. Alternatively, see the link below for other acceptable forms of identification. We accept cash, check, money order and major credit cards. We encourage you to call us prior to stopping in to ensure you have all required documentation.

All request forms should be signed and accompanied with a copy of your valid government issued photo ID.
Who Can Obtain a Death Record?
Death records are not public records and are only available to those who have a personal or property right interest (“property right” is defined as something that it is owned, tangible such as a car title or a property deed) with the decedent. To obtain a certificate, you must provide documentation of your personal or property right interest (i.e., an insurance policy).

Credit Card Fees:
Effective 01/04/2024, a credit card fee of 2.25% will be applied to all debit and credit card transactions. A minimum of $1.00 will be applied.
